Introduction
The dashboards on this page were developed in a partnership between the Meadows Mental Health Policy Institute and Path Forward. The dashboards present a span of healthcare claims data that represent healthcare activity of approximately two-thirds of individuals in the United States.
Data
Milliman Data: This dashboard was created by the Meadows Mental Health Policy Institute. All data presented here were received from Milliman and are subject to rules and restrictions contractually specified. Milliman's full report on the data is available here: URL
Data Suppression: Note that all cells with fewer than 13 observations in the Medicare and Medicaid data have been suppressed. To protect privacy, Centers for Medicare & Medicaid Services (CMS) requires suppression of values between 1 and 10. To prevent these values from being inferred, we have suppressed values between 1 and 12 and rounded all other values to the nearest 5. Calculations of per 100k insured people and units all used a value of 1 imputed into '< 13' cells, representing a conservative estimate of utilization. These suppression rules do not apply to provider counts or commercial market data.
National Provider Identifiers (NPI): CoCM claims experience was summarized by provider, separately for Original Medicare (Medicare Fee For Service), Medicare Advantage, and Medicaid. The NPI counts available for each insurance coverage type are included. Provider data was not reliable and is not reported for commercial data.
- For Original Medicare, the performing physician NPI (PRF_PHSYN_NPI).
- For Medicare Advantage, the rendering physician NPI (RNDRNG_PHYSN_NPI).
- For Medicaid, the servicing provider (SVCNG_PRVDR_NPI).
Provider Names and Practice Addresses:
- Provider names and practice addresses may change over time as providers update their information. We are utilizing the most recent information that is available via CMS's NPI Registry API.
- A small number of the NPIs that are reported on CoCM claims appear to no longer be active. No name or address was available in the latest NPI Registry for these NPIs.
Methods
Units: In healthcare claims, 'units' refer to the number of times a specific service, procedure, or item is provided, and these units are used to calculate the amount a provider can bill.
CPT/HCPCS Codes: CoCM patients and units reflect the usage of 99492, 99493, 99494, and G2214 CPT codes.
- 99492: Initial psychiatric collaborative care management, first calendar month, first 70 minutes
- 99493: Follow-up psychiatric collaborative care management, subsequent calendar month, first 60 minutes
- 99494: Psychiatric collaborative care management per calendar month, each additional 30 minutes
- G2214: Initial or subsequent psychiatric collaborative care management, first 30 minutes in a month of behavioral health care manager activities, in consultation with a psychiatric consultant, and directed by the treating physician or other qualified health care provider
Per 100k Insured People Calculation: All rates are reported based on the average number of individuals enrolled in healthcare coverage. All references to 'per 100,000 insured people' refer to 100,000 insured-person-years, where insured-person-years are calculated as the total number of months of healthcare enrollment divided by 12. This accounts for fluctuations in population enrollment within a year.
Region: Medicare regions are based on the provider's practice address and the Medicaid regions are based on the member's location.
Cross-Border Metropolitan Statistical Areas (MSAs): Note that some MSAs cross state borders and that we include an MSA in the estimate calculation for every state the MSA is a part of. Where such MSAs represent a meaningful proportion of the area population (e.g., Maryland), estimates may be impacted.
Dashboard Operational Notes
Please note that the following specifications may impact the user experience of the dashboard:
- The dashboards may load slowly when initially rendering the map, figures, and tables. The dashboards should automatically react to changes in the user selection options on the left side of the page, but reactions may be slow to load when many regions and years of data are selected.
- There are performance differences between browsers (e.g., Google Chrome, Apple Safari, Microsoft Edge) that may impact the functioning of the dashboard.
- If you are using the Google Chrome browser and the MSAs displayed on the map appear to be rendering incompletely or incorrectly, please try turning off graphics acceleration in your Chrome settings by toggling 'Use graphics acceleration when available' to off.
